New 49 CFR Part 40

November, 2017: The DOT Office of Drug and Alcohol Policy and Compliance issued a final rule which harmonizes 49 CFR Part 40 with the revised Dept. of Health and Human Services (DHHS) Mandatory Guidelines established for drug and alcohol  testing of federal employees. Highlights of this change, effective January 1, 2018

In 2010 The U.S. Department of Transportation (DOT) finalized some minor changes to their Alcohol Testing Form (what are the changes?). The revision date is 5/2008 and is reflected on the bottom left corner of the form.
DOT requires use of the new form for every DOT-mandated alcohol test as of 1/1/2011.  If you need to purchase new DOT forms please visit the “Forms & Log Books” accordion of our supplies page.
