New 49 CFR Part 40

In 2010 The U.S. Department of Transportation (DOT) finalized some minor changes to their Alcohol Testing Form (what are the changes?). The revision date is 5/2008 and is reflected on the bottom left corner of the form. 
DOT requires use of the new form for every DOT-mandated alcohol test as of 1/1/2011.  If you need to purchase new DOT forms please visit the Products Section of our website.
